No. And there’s no waiting period either. You’ll receive your account number the same day you join the plan. Your membership card(s) will be sent to you just a few days later.

What if my current dentist isn’t listed in the UDP Dentist Directory?
We’d be happy to discuss UDP participation with your dentist. Just ask them to call us at 1-800-894-8984.
What are the UDP membership guidelines for a family?
Family membership in UDP is unlimited, provided everyone shares the same Legal residence. Family members not living at the same address are not Eligible, except for children attending school.
What is the “liberal definition” of a couple, and what is UDP’s policy?
A couple is any two people who share a legal residence. UDP subscribes to the liberal definition for membership purposes. Therefore, a couple could be:
A single parent with a child
A person with a dependent
A person with a domestic partner
Same sex couples
What are the payment methods for membership?
Customers can select from monthly and yearly payments. All payments are renewed automatically.
How do I know what my dentist will charge me?
The patient fees are listed on the website for all the services. Any service not listed will be automatically discounted 20%.
I need a lot of dental work. Can I set up a payment plan with my dentist?
By all means, talk to your dentist. Because you’re required to pay them directly, it’s his or her decision.
I lost my I.D. card. How do I get a new one?
Just call our Customer Service Department. You’ll receive a replacement card In the mail.
How do I cancel my membership?
Please send us written notice of the date you wish to cancel, along with your I.D. card (verbal cancellations will not be accepted). If you don’t provide a Cancellation date, we will cancel your membership on the date we receive your Notice.
