THIS PLAN IS NOT INSURANCE. The plan provides discounts at certain dental care providers for dental services. Please see the fee schedule for a full list of discounts. The plan does not make payments directly to the providers of dental services. Plan members are obligated to pay for all dental services but will receive a discount from those dental care providers who have contracted with Universal Dental Plan. You may access a list of participating dental care providers at this website. Upon request the plan will make available a written list of participating dental care providers. You have the right to cancel within the first 60 days after receipt of membership materials and receive a full refund.
